Admission Requirements:
1.    Entrance Examination Result
2.    Cert. of Good Moral Character -Original and two (2) photocopies.
3.    Form 138 – A (HS Card) Original and two (2) photocopies.
(For Transferee : Transcript of Record and Honorable Dismissal – Original and three (3)
 Photocopies.
4.    Birth Certificate – Original ant three (3) photocopies
5.    1x1 ID Pictures (6pcs)
6.    Medical Results of the following
(Upon Enrollment)
-For BS Marine Engineering-
a. Hepa B Test – must be negative
b. Chest X-ray
c.  Ishihara (Eye) Test

-For BS Cruise Ship Management-
a. Hepa B Test -  must be negative
b. Chest X-Ray

Note: HAVE YOUR MDICAL EXAM AT THE JBLF DIAGNOSTIC CENTER
